On our mission to give the children of Chicago an eternal hope and a lasting future, CEF requires all individuals involved with CEF to fill out the Child Protection Policy Form for their and the children’s protection. Therefore, if you have any contact with any child at any event that is sponsored or co-sponsored with CEF or that receives any support from CEF, you must complete the Child Protection Policy Form in its entirety.
Download the Child Protection Policy Form by clicking the highlighted blue that is towards the bottom of this page. After it is filled out completely, either fax, mail, or email it to Chicago CEF. Please note that you must be approved by the Metro Director before having any contact with children under the auspices of CEF. Also note that this form must be filled out every five years to guarantee up-to-date information.
